ENGAGEMENT TYPE: Fractional / Part-Time Contract (scalable to Full-Time)
LOCATION: Remote with occasional travel to in-person meetings as needed
TIME COMMITMENT: ~20 hrs/week to start; flexible based on program load

1. WHO WE ARE
Community Health Patient platform is a community physician and patient engagement organization dedicated to connecting specialty patient communities to health education, clinical research, and life-saving treatment. We bridge the gap between patients, physicians, and pharmaceutical partners through culturally competent programs, patient-facing webinars, and community outreach.
This role sits at the intersection of health storytelling, medically accurate education delivered by knowledgeable, trusted physicians, digital strategy, and mission-driven content. It is ideal for a creative, strategic, medically-savvy, lead generation health writer and content owner who is passionate about engaging patients who are often overlooked and underserved.

2. WHY THIS ROLE EXISTS
We are expanding our patient engagement infrastructure to drive more patients into our Community Health network through compelling content across digital and social channels. This person will serve as the creative and strategic engine behind our content pipeline — from physician-approved educational materials to patient video stories to short-form social content — with a clear mandate to generate patient leads and grow our community.

3. OUTCOMES — What You Will Accomplish
In keeping with our Hiring for Performance methodology, the following outcomes define success in this role.
Outcome 1: Develop Physician-Approved Patient Education Content
Within 60 days, develop and deliver the first complete physician-approved patient education presentation for a Community Health webinar, including:
- Independent research on patient-relevant health topics (e.g., hepatitis B, fatty liver disease/MASH, immune conditions such as lupus, multiple oncology conditions)
- Collaboration with physician speakers to ensure medical accuracy and speaker sign-off
- Coordination with the Community Health tech team to execute the live or on-demand webinar
- Ongoing iteration as new webinar topics are added to the calendar
Outcome 2: Social Media Strategy That Converts Patient Leads
Within 30 days, present a social media content strategy; within 45 days, have it live and executing, including:
- A managed social content calendar across Facebook, Instagram, YouTube, TikTok, and LinkedIn
- Repurposing long-form webinar content into short-form static and video assets
- Clear conversion pathways: content → registration → patient lead captured
- Collaboration with the Coordinator on organic and paid post execution
- Monthly reporting on reach, engagement, and lead conversion metrics
Outcome 3: Active Online Patient Community Engagement
On an ongoing basis, directly engage with prospective patients in the digital spaces where they gather, including:
- Monitoring and participating in condition-specific online support groups (Facebook groups, Reddit communities, patient forums)
- Identifying and engaging new patients through social media channels in a compliant and human way
- Building relationships that drive patients to Community Health's network and resources
- Maintaining a pipeline log of prospective patient contacts and conversion status
Outcome 4: A Living Patient Voice Content Library
Within 90 days, establish a pipeline and library of patient video stories that features real patients sharing their health journeys, including:
- Sourcing and recruiting patients from Community Health's network, support groups, and social channels
- Creating culturally sensitive interview question guides tailored to each patient's condition and background
- Facilitating video interviews in a way that builds trust and draws out compelling narratives
- Editing raw footage into polished snippets (30–90 seconds) for Community Health and social platforms
- Maintaining a content calendar that keeps patient voices consistently visible across channels
Outcome 5: LinkedIn as a Community Health Growth Channel
Within 45 days, deliver and execute a LinkedIn strategy that elevates Community Health's visibility among patients, healthcare providers, and potential pharma/biotech partners, including:
- A defined content cadence (thought leadership, program highlights, patient stories)
- Community engagement (HCP audiences, health advocacy organizations, health systems)
- Measurable growth in followers, post impressions, and inbound inquiries

4. COMPETENCIES — Who You Are
We are looking for an 'A Player': someone who not only can do the job but will do the job with excellence and fits who we are as an organization.
What You Must Have
- Health or patient content experience — You understand how to communicate health information accessibly, accurately, and compliantly. Experience in community health, pharma, or health nonprofits is a strong plus.
- Storytelling instinct — You can turn a patient's lived experience into content that moves people to act. You understand narrative arc, emotional resonance, and the difference between information and engagement.
- Digital and social media fluency — You are comfortable and current across Facebook, Instagram, TikTok, LinkedIn, email, and SMS. You know what performs and why.
- Video production capability — You can facilitate, record (or direct), and edit video to a professional standard — at minimum, short-form cuts — without always needing external production support.
- Lead generation orientation — You think in funnels. Every piece of content you create has an intentional purpose: attract, engage, and convert a patient _on a particular journey_ into our network.
- Relationship-building with patients — You can earn the trust of someone navigating a health condition, from an immigrant community, who may be skeptical of institutions. You are empathetic, culturally competent, and persistent.
- Self-direction — This is a fractional role. You will manage your own time, set your own workback timelines, and deliver. You do not need to be managed.
- Follow all compliance guidelines
What Would Set You Apart
- Experience with physician or HCP collaboration in content development
- Working knowledge of health literacy principles and plain-language communication
- Familiarity with CRM and patient lead tracking tools
- Experience with paid social campaigns, particularly lead-gen objectives on Meta and YouTube
