Fractional Head of Product Engineering – AI Healthcare Platform
Contract / Fractional | 90-Day Initial Engagement | 15–25 Hours per Week
Scrivas is looking for a senior, hands-on engineering leader to take technical ownership of SAI, our AI-powered clinical documentation platform.
This is not a traditional Fractional CTO advisory role.
We are not looking for someone to primarily sit in meetings, provide high-level recommendations, or manage from a distance.
We need someone who can get into the code.
About Scrivas & SAI
Scrivas was founded in 2012 by Dr. Fernando Mendoza, a practicing pediatric emergency physician, to address the growing documentation burden on physicians.
After more than a decade of physician-led clinical documentation experience, Scrivas is applying that knowledge to SAI, an AI-powered clinical documentation platform designed around real physician workflows.
SAI currently has a React web application and Flutter mobile applications for iOS and Android and is in beta with users. The platform has been developed with external development partners.
We are now looking for an experienced technical leader to represent Scrivas' interests on the engineering side and help us determine what the platform needs to successfully move forward.
Your 90-Day Mandate
Your initial assignment is straightforward:
Understand what we have. Stabilize it. Establish technical ownership. Tell us what we need next.
During the first 90 days, you will be expected to:
- Conduct an independent technical assessment of the existing SAI platform and codebase.
- Review the React/TypeScript web application and Flutter mobile applications.
- Understand the current architecture, APIs, backend dependencies, AI/LLM integrations, development environments, and deployment processes.
- Identify the root causes of technical issues affecting beta users.
- Personally troubleshoot complex or high-priority technical problems.
- Establish a prioritized plan for defects, technical debt, performance issues, and architectural risks.
- Review code and pull requests produced by external developers.
- Evaluate the quality and capabilities of our current development resources.
- Review vendor estimates and proposed technical solutions.
- Improve testing, monitoring, logging, error tracking, code review, CI/CD, and release processes where needed.
- Establish appropriate engineering standards and documentation.
- Serve as Scrivas' senior technical representative with our external development partners.
- Provide leadership with a recommended 6–12 month engineering roadmap.
- Recommend what engineering capabilities should remain outsourced versus eventually be brought in-house.
We Need a Builder, Not Just an Advisor
This distinction is important.
We are not looking for a Fractional CTO who primarily attends meetings and provides strategic advice.
We need someone who can open the repository, understand unfamiliar code, diagnose why something isn't working, challenge a proposed technical solution, review the fix, and personally contribute when the situation requires it.
You should be comfortable moving between:
Code → Architecture → Debugging → Vendors → Product → Executive Leadership.
There will certainly be strategy involved, but that strategy needs to come from someone who understands the product at the code and architecture level.
Technical Background We're Looking For
Candidates should have significant production engineering experience, including:
- Advanced React and TypeScript experience.
- Strong modern front-end architecture experience.
- Experience inheriting and improving an existing production codebase.
- Experience diagnosing difficult production issues.
- Experience with REST APIs and frontend/backend integrations.
- Experience with cloud-based SaaS applications.
- Strong knowledge of testing, monitoring, CI/CD, deployment, and production support.
- Experience building or supporting AI/LLM-enabled production applications.
- Experience reviewing architecture and production-quality code.
- Technical leadership experience.
- Experience managing or overseeing third-party/offshore development resources.
- Ability to communicate technical issues clearly to non-technical leadership.
Strongly preferred:
Flutter/iOS/Android experience, healthcare technology, HIPAA-regulated applications, clinical workflows, Python environments, RAG/LLM architectures, AI evaluation, and startup/early-stage SaaS experience.
We do not expect one person to check every preferred box.
Who This Is NOT For
This engagement is probably not a fit if you:
- Have moved completely away from hands-on engineering.
- Primarily provide Fractional CTO strategy/advisory services.
- Primarily manage projects rather than engineering.
- Need a team of junior developers to perform the technical assessment for you.
- Have AI strategy experience but limited experience shipping production software.
- Are uncomfortable inheriting someone else's code.
- Are looking for a full-time management position rather than a fractional engagement.
Engagement Structure
We anticipate:
Initial term: 90 days
Commitment: Approximately 15–25 hours per week
Structure: Independent contractor/fractional engagement
Potential: Extension based on the needs of SAI following the initial assessment and stabilization period
We are intentionally starting with a fractional engagement rather than immediately building a large internal engineering organization.
At the end of the initial engagement, we want to have a clear understanding of SAI's technical condition, the capabilities of our existing development resources, how much engineering capacity we actually need, and which capabilities should remain outsourced versus eventually come in-house.
